Who's Eligible?
Eligibility for CHSP is based on your age and support needs. If everyday tasks have become more difficult and a little support could improve your health and wellbeing, CHSP may be right for you.
You may be eligible if you are:

65 years and older (or 50 years or older for Aboriginal or Torres Strait Islander people)
-
50 years or older (or 45 years or older for Aboriginal or Torres Strait Islander people) on a low income and homeless, or at risk of homelessness
An aged care assessment will determine your eligibility for CHSP services, including specific modifications and repairs.
How Much Will It Cost?
You may be asked to make a contribution toward the total cost, based on the type of services provided and your financial situation.

How Our Process Works
At LifeTec, we make it easy to access the support you need for CHSP Minor Home Modifications.
Here's how we work for you:

Provide your CHSP Home Modification referral code and join the service queue

You will be allocated to an Occupational Therapist

Your Occupational Therapist will asses your home

LifeTec will seek a quote from our suppliers and builders

Set up and ImplementLifeTec will seek a financial contribution from you, along with your approval for the quoteation
