End-Of-Life Pathway

From 1 November 2025, the End-of-Life Pathway will be introduced as part of the Australian Government's Support at Home Program. This new pathway is designed to help older Australians with a life expectancy of three months or less remain at home, surrounded by familiar comforts and loved ones, for as long as possible.

At LifeTec, we are preparing to deliver compassionate, practical support through this pathway—ensuring individuals and families receive the care they need during this deeply personal and important stage of life.

What Is the End-of-Life Pathway?

The End-of-Life Pathway offers time-limited, intensive in-home support for older people who wish to stay at home during their final months. This funding complements existing palliative care services and aims to:

  • Maximise comfort and dignity

  • Provide tailored, home-based care aligned with the person's wishes

  • Support carers and families throughout the process

  • Minimise unnecessary hospital or residential admissions

 

Eligible participants will receive up to $25,000 in funding over a 12-week period (with a 16-week spending window). This includes access to personal care, domestic assistance, general nursing, and other aged care services needed to remain safely at home.

Who's Eligible?

You may be eligible for the End-of-Life Pathway if:

  • A medical or nurse practitioner has confirmed a life expectancy of three months or less

  • You have a score of 40 or less on the Australian-modified Karnofsky Performance Status (AKPS) scale (indicating a need for daily support)

 

You do not need to be an existing Support at Home participant. A high-priority assessment can be arranged via My Aged Care to access the pathway promptly.
