Assistive Technology and Home Modifications (AT-HM) Scheme

As part of the Australian Government's Support at Home Program, the Assistive Technology and Home Modifications (AT-HM) Scheme gives older Australians access to practical, tailored solutions that support safe, independent living.

What Is the Assistive Technology and Home Modifications (AT-HM) Scheme?

From 1 November 2025, Support at Home participants will be able to access assistive technology and home modifications without needing to save up funds from their quarterly budgets. Funding tiers will be approved as part of an individual's Support at Home assessment, with access to:

Assistive Technology (AT)

Assistive technology includes devices or products that help people do tasks more easily or safely, or support them to continue daily activities independently.

Examples include:

  • Walking aids such as walking sticks, frames, or wheelchairs
  • Bathing supports like shower chairs and bath boards
  • Toileting aids including commodes and bedpans
  • Personal alarms and smart monitoring systems
  • Adaptive kitchen tools and communication devices

Home Modifications (HM)

Home modifications are physical changes made to improve safety, accessibility, and ease of movement within the home.

Examples include:

  • Grab rails and handrails in bathrooms and stairways
  • Internal or external ramps and stair lifts
  • Non-slip materials for floors and stairs
  • Bathroom adjustments (e.g., handheld showers, accessible fixtures)
  • Threshold ramps and step alterations

Some participants may also be approved for more complex modifications, such as:

  • Bathroom redesigns to improve accessibility
  • Widening doorways or passages for wheelchair access

Who's Eligible?

The AT-HM Scheme is available to individuals who:

  • Are aged 65 years or over (or 50+ for Aboriginal or Torres Strait Islander people)
  • Have been assessed and classified under the Support at Home Program
  • Have identified functional limitations that can be addressed through AT or home modifications

You'll need to go through a My Aged Care assessment to access this scheme.
