Restorative Care Pathway

From 1 November 2025, the Restorative Care Pathway will become part of the Australian Government's new Support at Home Program. This pathway is designed to help older people regain function, build confidence, and maintain independence after a health event, change in condition, or early signs of decline.

Unlike traditional care that focuses on managing decline, the Restorative Care Pathway empowers participants to restore and maintain independence, enabling them to keep doing the things they enjoy for longer.

What Is the Restorative Care Pathway?

The Restorative Care Pathway is a short-term, intensive support program that offers up to 12 weeks of coordinated allied health and nursing services. An additional 4-week extension may be available depending on individual goals and progress.

This pathway is:

  • Person-centred and goal-oriented
  • Focused on early intervention and reablement
  • Delivered through multidisciplinary, team-based care
  • Inclusive of participants' families and support networks
  • Built around the principle of "doing with" rather than "doing for"

It aims to:

  • Optimise, maintain, or improve daily function
  • Delay or reduce reliance on ongoing care
  • Prevent or reverse physical, functional, or cognitive decline
  • Improve quality of life and self-reliance

Who's Eligible?

You may be eligible for the Restorative Care Pathway if you:

  • Are recovering from illness, injury, or recent hospitalisation
  • Are experiencing early signs of physical or functional decline
  • Have been newly diagnosed with an age-related condition
  • Want to avoid or delay the need for higher levels of care
  • Are already receiving ongoing services but need short-term, intensive support to restore function

Your access will be determined through an assessment via My Aged Care, and funding will come from your Restorative Care budget under Support at Home.
