How the CHSP Home Adjustments Pathway Works
Already had your My Aged Care assessment? Here is what happens next.
My Aged Care assessment
Your My Aged Care assessment determines your eligibility for CHSP services.
If you are approved for CHSP Home adjustments, you will receive a referral code to give to your chosen provider.
LifeTec OT home assessment
A LifeTec Occupational Therapist visits your home to understand your needs, assess safety risks and identify suitable adjustments.
Your OT then prepares recommendations and a plan for the proposed work.
Quotation and trusted installation
LifeTec obtains quotations and explains the proposed work, available funding and any client contribution. There is no obligation to proceed.
If you choose to continue, the approved work is completed by trusted, licensed contractors with guidance from your Occupational Therapist.
Live more safely at home
Your Occupational Therapist checks that the completed adjustments meet your assessed needs.
The goal is to help you move around your home more safely and maintain your independence.
Who May Be Eligible?
You may be eligible to access CHSP services if you:

Are aged 65 years or older

Are an Aboriginal or Torres Strait Islander person aged 50 years or older

Are homeless or at risk of homelessness and aged 50 years or older
-
Need entry-level support to continue living safely and independently at home
Eligibility is determined through a My Aged Care assessment.

Costs and Contributions
CHSP services are subsidised by the Australian Government, but you may be asked to contribute towards the cost of your home adjustments.
LifeTec will explain the quotation, available funding and any contribution before work begins.
