Who's Eligible?
You may be eligible for CHSP Minor Home Modifications if you:

Are 65 years or older (or 50+ for Aboriginal and Torres Strait Islander people)

You are on a low income and at risk of homelessness

Have difficulty completing everyday tasks safely

Have an aged care assessment recommending CHSP services
An aged care assessment will determine your eligibility for specific modifications.
